What is Companeros en la Jornada?
Companeros en la Jornada, is an outreach program of Hospice at The Chaplaincy. It is designed to meet the unique needs of the Hispanic Community in Benton and Franklin counties and the surrounding areas.
The goals of the program are to:

Provide culturally sensitive hospice services to the Hispanic population with emphasis on respect for diverse belief systems and cultural practices

Provide excellent care to those with serious illnesses

Support families and care givers
The Chaplaincy believes that the ability to interact with people in their primary language improves communication, creates an atmosphere of trust and leads to more positive outcomes.
Bi-lingual employees are trained to work with patients and families to develop care plans that incorporate a patient’s culture and belief systems. We also offer literature and all critical documentation in Spanish to ensure clarity and understanding. |
The Circle of Care
When faced with a serious illness, Companeros en la Jornada offers the patient and family a “circle of care” which consists of the medical director, a nurse, nurse assistant, social worker, chaplain, volunteer, grief support, and your family doctor. This team works with families and patients to identify problems, set goals and keep the patient as comfortable and symptom-free as possible. Care is provided in the patient’s home, wherever that may be. |

Medical Director: Works with the patient's family doctor and oversees the hospice care.
Nurse: Evaluates the physical condition and, after consulting with the family offers suggestions for intervention such as treatments or medications.
Nurse Assistant: Helps with bathing and grooming to keep the patient clean and comfortable.
Social Worker: Provides support, problem-solving, coping strategies and connections to community resources as wanted or desired by the patient and family.
Chaplain: Focuses on spiritual and emotional needs of the family and patient, giving assurance and comfort at this difficult time and arranging sacraments as needed or desired.
Hospice Volunteers: Sit with the patient to allow family and caregivers a break.
Grief Support: After a loved one dies, grief support is available for all ages - children through adults.
Family Doctor: The patient's physician continues to direct their care. |
